ASIA North Poplar Application Form
Our application form is used when a family wants to apply for a spot for their child(ren). Once an application has been approved we will contact you and ask you to complete the ASIA North Poplar Registration Package which can be found under the Registration drop down menu or picked up at the school office.
Please note the attached application has 3 sections to be completed. Once completed it can be dropped off at the school along with a copy of your child's most recent report card. We will be happy to take a photocopy for you.
